 Scheme terms and conditions and required documents

1. Online/UDD Medical Certificate of Civil Surgeon in case of mental retardation (40 percent)
2. Annual income of the beneficiary should be Rs.1,00,000/- / Beneficiary should be below poverty line) Certificate of annual income of the family in the year 2024-25 from Tehsildar
3. Government / District Council / Pt. S. Self-declaration of not availed of level scheme
4. Only complete proposals will be accepted after scrutiny/scrutiny of proposals through Group Development Officer Panchayat Samiti.
5. Beneficiary will be selected in Social Welfare Subject Committee.
6. The cost of the scheme is 25 GP. Cess 2235 Social Welfare Scheme No. 2235-101-33 Financial assistance to mentally challenged persons (limit Rs. 25000/-) will be provided from this scheme.
